5. Relevant Certificates with one Photocopy (Caste Certificate for Reservation, Income Certificate for Fee Concession, NCC, NSS, PH, Sports, Ex Servicemen, Certificates of the HSE State Level A Grade Holders of Fine Arts, etc.)

6. T C from last studied Institution (Mandatory for Admission) with one photocopy of the same.

7. Conduct Certificate within 6 months (Mandatory for Admission)

| Documents to be submitted at the time of admission: |

1. Allotment Memo (Download from applicant log in).
|
|
2. Duly filled and signed Application form (Candidates availing of the community quota reservation should obtain the signature of their respective parish priest in the prescribed space of the application form) *

3. Original transfer certificate (TC) along with one self-attested copy.
|
|
4. Conduct Certificate obtained within 6 months.
|
|
5. Degree Consolidated Mark list (Original +2 copy)
|
|
6. Provisional/Degree certificate (Original + 2 copy) if available
|
|
7. Original Plus-two mark list and one self-attested copy of the same.
|
|
8. Original SSLC certificate and a self-attested copy of the same. *.

9. Caste Certificate (If availing seat reservation).

10. Income Certificate (If applied for Fee Concession).
|

11. Original and self-attested copies of certificates to avail of the bonus marks (such as NCC, NSS).

12. Migration Certificate (for students completed degree from other Universities in Kerala).
|
|
13. Equivalency /recognition certificate (for students completed degree from Universities outside Kerala).

14. CAPID Print out (If already registered in Calicut University).

NB: Please take sufficient number of photocopies of the certificates before submitting and keep with you for further use
|

1. The submitted certificates will get back only after the completion of the programme or at the time of the TC.
|
|
2. Those who got seat in community quota should bring the printout of application form duly signed by the respective parish priest.
|
|
3. Second Language selected and marked at the time of submission of Application can only be changed at the time of interview.
|
|
4. The candidate will loose the seat because of the following conditions:- Incompletion or wrong submission of the Application Form, being absent at the time of the interview date or non production of documents mentioned in the admit card etc.
